Strong quakes hit Japan's jumpy northeast

A SERIES of strong earthquakes rattled northeast Japan today, keeping residents on edge more than a week after a devastating tremor and tsunami battered the region.
Two 6.6-magnitude tremors and one 6.4 quake struck within two and a half hours off Japan's tsunami-stricken northeast coast, starting from 6.18pm (AEDT), the US Geological Survey reported.
There were no reports of casualties or damage and no tsunami warnings.
A record 9.0 magnitude which spawned a towering tsunami battered Japan's northeast coast on March 11, leaving more than 21,000 people dead or missing and triggering a crisis at a nuclear power plant in Fukushima.
Around 20 per cent of the world's most powerful earthquakes strike Japan, which sits on the "Ring of Fire" surrounding the Pacific Ocean.

It was centered around Shizuoka prefecture, and had a magnitude of about 6.0 just some moments ago. Local news is reporting some power outages have initially occurred following today’s Shizuoka, Honshu earthquake. A strong earthquake with a preliminary magnitude of 6.0 jolted Shizuoka Prefecture and its vicinity, including Tokyo, on Tuesday evening. The US Geological Survey put the magnitude at 6.1 and said it had been preceded a few minutes earlier by another 5.8-magnitude tremor.It rumbled from a relatively shallow depth of 10 kilometres, with a magnitude of 6.0. The epicenter was inland, so they are saying there is no tsunami risk which is a little positive news in this worst time for Japan history. A large number of citizens have left the city. Tokyo residents had been told Tuesday that the danger posed by radiation leaking from the Dai-ichi nuclear plant in the Fukushima prefecture some 240 kilometres northeast of the city was minimal.

Japan Earthquake Disaster, Accident or Incident? - Posted 22nd March 2011
I was with a group of people recently watching the news about the Japan earthquake and the damage caused at the nuclear plants. A discussion developed as to whether the event should be described as an accident or incident, or is it both?
Definitions were quickly looked up on Google, for example:
Accident:
From ROSPA FAQs - “An unplanned, uncontrolled event which has led to or could have led to injury to people, damage to plant, machinery or the environment and/or some other loss."
It also said the HSE provides the following definition - “An undesired circumstance(s) which gives rise to ill health, injury, damage, production losses or increased liabilities."
Incident:
From HSE website – “A major incident is defined as a significant event, which demands a response beyond the routine, resulting from uncontrolled developments in the course of the operation of any establishment or transient work activity”.
Could some decisions have been made in planning that could have controlled events? Or even avoided them? Opinions were divided, but it gave for interesting debate and thought.
